Alzheimer's Association, Greater Maryland Chapter

  • Support Groups - join a support group or become a support group facilitator and connect with others online.
  • Education Programs - free educational programs for those with Alzheimer's and their families.
  • Multilingual Information - Alzheimer's educational materials in Chinese, Korean, and Spanish.
  • Lending Library - free books, DVDs, and videos available for rent. Call (800) 272-3900 for more information.
  • Local Resources and Referrals - updated information on local resources such as home/adult day care, lawyers, transportations, etc.
  • Local Research Trials - learn how you can make a difference by participating in clinical trials and studies to help find treatments for Alzheimers.

  • Asthma 101 - workshop on the basics of asthma and how to manage it.
  • Better Breathers Club - community-based educational opportunities and support to those with chronic lung disease and their families.

  • Support Groups - patients and caregivers can find support through these local groups

Breast Cancer Network of Strength

  • Support Groups - meetings are held regularly for breast cancer patients, survivors and all who are concerned with breast cancer.
  • Community Outreach - "A Day For You" participants attend an all-day event and learn about earlier detection methods, are taught breast self-examination (BSE) and receive clinical breast exams.
  • Workshops - breast health awareness workshops are presented in a concise 60-minute format.
  • Local Resources at Your Fingertips - resource guide to help you, your family, friends, and health care team find resources and information in one place.
  • Your Shoes 24/7 Breast Cancer Support Center - get immediate emotional relief; talk to a breast cancer survivor.
  • Partner Match Program - provides support and education for people while they are supporting a wife, partner or other loved one through the disease.

  • Adult Day Services - provides daily social and medical support for adults with age-related impairments.
  • Assistive Technology - center that provides the latest in low and high technology solutions, minimizing the functional impact of a person’s disability.
  • Child Development Centers - early intervention and education, full-day childcare, parental support, and intergenerational activities programs.
  • Family Caregiver Support - meets the educational, referral, and support needs of families carrying for a senior or child with a disability.
  • Respite Services - provides the caregivers with much needed time off from their care giving responsibilities to relieve stress and prevent burnout.
  • Home and Community Services Program - the Easter Seals Home and Community Services Program is designed to increase independence, function,
    and productivity of persons with disabilities of all ages through direct service and support/training.

  • Independent Living Skills Training - provides individuals with disabilities an opportunity to make appropriate choices and learn the skills which will assist them in achieving the highest level of independence possible.  Call 301.846.7811 or send an e-mail to advocate@thefreedomcenter-md.org
  • Advocacy - The Freedom Center provides advocacy in the areas of housing, transportation, civil and human rights, education, employment, and other life situations.  Self advocacy is action taken on one's own behalf to protect or improve the individual's life circumstances.  Individual advocacy is assistance given to a person which is intended to improve both the person's life circumstances and the person's self advocacy skills.  Systems Advocacy is activity which has the effect of altering laws, regulations, or practices that impact large numbers of people.  Call 301.846.7811 or send an e-mail to advocate@thefreedomcenter-md.org
  • Nursing Home Transition - provides advocacy, support, and services to facilitate the least restrictive environment and give opportunity to individuals to choose to live in the most integrated setting.  Individuals who are living in nursing, assisted living, or state facilities will be linked to every services available to maximize independence in their own homes for full integration into the community.  Call 301.846.7811 or send an e-mail to advocate@thefreedomcenter-md.org

  • Voice For Freedom - offers services to nursing home residents who are not able to communicate.  Through this program, people with disabilities who live in nursing homes and cannot communicate verbally may be evaluated for an augmentative communication device and trained on how to use the device that is purchased for them through funding from the Verizon Foundation.  Call 301.846.7811 or send an e-mail to advocate@thefreedomcenter-md.org

Hospice Network of Maryland

  • Finding Hospice Care - find hospice care in Maryland by County
  • Palliative Care Hotline - any physician who has a pain management question can call and receive the immediate expertise of a physician who has been trained and certified in the specialty of hospice and palliative care medicine.  For physicians only: 1.800.492.3805

Lupus, Mid-Atlantic

  • Lupus Information - information on lupus, diagnosis and treatment
  • Online Support Groups - receive the support you need without leaving your home! Our online groups are facilitated by a licensed clinical social worker and you can log-on anytime, day or night, weekends or holidays
  • Quality of Life Programs - designed to assist lupus patients facing financial hardship who cannot afford personal care items pertaining to their lupus. This is a program of last resort - to apply, call 410.337.9000 or 800.777.0934

Moveable Feast

  • Daily Home Delivered Meals - provides meals for clients who are homebound with HIV/AIDS or breast cancer in Baltimore City and 13 Maryland counties
  • Meals for a Week - available for people living with AIDS and women undergoing treatment for breast cancer who are not fully homebound but are unable to return to work, and cannot shop for themselves
  • Medical Transportation Program - wheelchair-accessible transportation is provided to and from medical appointments.  Destination and client must both fall within Baltimore City.  Call 410.327.3420 ext.15
  • Nutritional Counseling - full time registered dieticians provide nutritional counseling and education to clients and community groups.  Diets are specifically tailored to meet a client's nutritional needs.  Contact Jinee Burdg at 410.327.3420 ext.12
  • People on the Move - shuttle service providing transportation among sites regularly used by homeless people in Baltimore City.  Call 410.327.3420 ext. 15

National Kidney Foundation of Maryland

  • KEY Screenings - KEY (Kidneys: Evaluate Yours) screenings are held throughout our service area, identify early markers for high blood pressure, diabetes, and kidney disease

 National Multiple Sclerosis Society, MD Chapter

  • Durable Medical Equipment and Loan Closet - assistance with gaining access to equipment such as walkers, scooters, supplies and more; contact 1.800.344.4867 for further assistance
  • Emergency Financial Assistance - the Maryland chapter offers a one-time emergency financial assistance to help with eviction notices or shut-off utility bills to help maintain personal safety and wellbeing of people affected by MS
  • For People Newly Diagnosed - if you or someone close to you has recently been given a diagnosis of multiple sclerosis, you probably have a lot of questions and concerns
  • Medical Transportation - assistance for transportation needs for medical appointments and some chapter programs; contact 1.800.344.4867 for more information
  • Research - over 440 society funded research investigations in progress around the globe, including 130 clinical trials
  • Wellness -staying well is a multi-faceted task and an important goal for everyone, especially those living with multiple sclerosis
